Yes this is very bizarre. I do wonder why the woman has never applied for a British passport, when she clearly lives in the UK and yet she holds a Canadian one and doesn't live there? Something is amiss. Perhaps she knew that she was not eligible for a British passport?
I'm sure she will get her British Nationality, albeit it will take time and money. This kind of reminds me of a friend I had back in British Columbia, she was in her 60's and had lived nearly all her life in Canada. She was of German decent but born in Mexico. Anyway, she was told by immigration that she had no right to stay in Canada, as they could find no record of her landing here in the first place. She had to go through the whole residency thing over again.
I guess back then records were not computerized, everything was on paper, in hard copy and much of those copies have been lost or destroyed over the decades. Too bad for the legitimate people who cannot now find their records.
